Patient Management
Cyfluent is a complete outpatient EHR system that has continuously updated clinical workflows based on published treatment standards. Its patient and provider management structure allows for the organization of complex medical communities and the easy identification and operations of patient files. The summary page displays basic information about the patient and contains configurable aspects of patient longitudinal data such as Allergies, Prescriptions, Problems, Patient Health History, Family History, Personal Habits, Contacts, Patient Alerts, Body Measurements, previous Vitals, previous Encounters, Vision, and Quality Indicators.

Clinic Processes
In patient engagement, Cyfluent supports all treatment pathways, chronic disease management, prescription management, computerized physician order entry (CPOE), interoperable encounter documentation, clinical flowcharts, structured and unstructured medical notes, and nutritional notes. Each encounter mimics the paper note for ease of use, making all sections of a progress note visible from one screen. Multiple Encounters for a patient can be opened simultaneously to quickly review previous problems. Clinicians can document quickly and efficiently through customizable templates that enables them to compose sentences and paragraphs according to selections and entries made during the encounter.

Lab Functions and Prescriptions
Electronic orders and results management for prescriptions, labs and procedures are all included and accessible from inside the encounter. eRX for prescriptions operates with one click and enhances accuracy and patient satisfaction with automated checks for drug contraindications, dosing alerts, and generics. Our electronic prescription now includes secure Electronic Prescribing for Controlled Substances (EPCS). This solution is DEA certified and includes two-factor authentication, electronic submission, and audible events tracking.

Telehealth
Cyfluent further has a robust synchronous and asynchronous telehealth capability through the Patient Health Record (PHR) Portal. The system can further directly connect with and pull information from remote home telehealth devices. These devices include equipment from leading vendors such as Cardiocom, AMC, Authentidate, American Telecare, Bosch Healthcare, Visual Telecommunication Network, and Viterion TeleHealthcare. We can also use the 2net™ Platform from Qualcomm Life which goes through an in-home Wi-Fi hub as a backup connectivity solution.

Patient Records and Mobile App Interoperability
Cyfluent mobile app and PHR allow patients to manage their medical information via the Web. Our Data Integration technology features connection of hospital systems operating within the enterprise, the delivery of health data to and from each system that is “plugged” in, the creation of CyChart enterprise data repository, compatibility with XML, HL7 FHIR, X.12, CCR, TXT, HTML, SQL, PDF, DICOM and other image formats. These exchanges support clinical assessments, medication reconciliation, care plans, order management, Clinical Data Exchange (CDE), encounter documentation, physician web access, patient web access, and notes access. Cyfluent can further pull information from multiple devices, such as smart watches, portable sonograms, EKGs, etc. Some of our providers have used Cyfluent to collect data from patients who regularly monitor their vitals remotely via telehealth automation intervals. The most common protocol for small device interfacing is Bluetooth, but we also can interface via direct Wi-Fi and USB, among other approaches.
